Financial Assistance Forms |
![]() This packet contains information about USDA income guidelines for 2019-2020 to be used in determining whether your family is eligible for free or reduced price meals. It also includes the District 70 Waiver of Fees form and the Application for Free or Reduced Meals. Download the packet, print the forms, then complete them and bring to the District Office with appropriate documentation.
