District 70 families are invited to a Prioritization of Urgency for Need of Services (PUNS) initial sign-up night being hosted by D128 on Feb. 29.
PUNS Night will run from 4-7 p.m. on Feb. 29 at the D128 Pathways Transition Center, 904B S. Milwaukee Ave., Libertyville.
To sign up for a 30-minute time slot (first come, first serve basis) to complete the PUNS registration, contact Christi Steiner at email@example.com. Spanish-speaking case managers will be available.
PUNS is a database of Illinois infants, children, adolescents, and adults with developmental disabilities who have needs for developmental disability services or supports. The PUNS database helps the state identify and plan for a child's immediate and future needs.
Children age 3 and up may be registered for PUNS. The earlier your child is on the PUNS database the better. This is the only way the State of Illinois will know that your child needs or will need services (i.e. funding). There is no guarantee that you will receive funding; however, if you are not on the PUNS database, you will not have the chance to be chosen for services (except in a crisis.)
On Thursday, Feb. 29, Community Alternatives Unlimited (CAU) will bring case managers to meet face-to-face with the primary caregivers and/or guardians and their children/young adults with special needs. The interview will last approximately 30 minutes.
Please bring the following for your PUNS appointment:
1. The Student,
2. Copy of Social Security Card,
3. Copy of Medicaid/All Kids card (if you have one),
4. Copy of current IEP and/or Psychological Evaluation to show eligibility for special education services (we can have this information printed for you that evening).
If you have already signed up for the PUNS list, you can update each year by calling 773-867-4000 and making an appointment to update over the phone. You do not need to make an in-person appointment when you are updating each year.
