Student health is very important in Libertyville District 70. To support students, a full-time registered nurse is on staff at each school. The nurses help students with bumps, bruises, health emergencies, and ensure student health records are up-to-date.
The school code of Illinois, Section 665, requires that each student entering school for the first time (kindergarten) and students entering sixth grade, along with transfer students, must submit evidence of the various examinations.
New students entering District 70 for the first time, kindergarten, 6th grade students, and students electing to participate in sports.

The physical examination must be dated within one (1) year before the first day of school and must be completed on the State of Illinois Department of Human Services Certificate of Child Health Examination. Signed examinations are due at the time of registration or prior to starting school. The physical examination form requires three signatures to be considered "in compliance."

D70 families should make arrangements with physicians as soon as possible to ensure their child(ren) will have the required exams and immunizations before school starts.
Students enrolling at the beginning of the school year who do not meet these requirements will be excluded from school attendance per district policy. Medical and religious objections to any physical examination or immunization must be submitted in writing and must contain the required information.
The following are the immunization requirements for the State of Illinois:
POLIO - Four (4) doses with the last dose received on/after the fourth (4th) birthday. Minimal intervals between doses are 4 weeks (28 days), the 4th dose is given at least 6 months after the previous dose.
DTP/DTaP/Td- For Kindergarten, and First (1st) grade: four (4) or more doses of DTP/DTaP, with the last dose being a booster and received on/after the fourth (4th) birthday. Other grades: Three (3) or more doses of DTP/DTaP or Td; with thelast dose qualifying as a booster if received on/after the fourth (4th) birthday. Minimal intervals between doses no less than four (4) weeks apart, last dose at least six (6) months after the previous dose.
HEPATITIS B - For students entering 6th grade: Three (3) doses: Interval of at least four (4) weeks between first (1st) and second (2nd) dose (28 days), interval of at least eight (8) weeks between the second (2nd) and third (3rd) dose (56 days), interval of at least sixteen (16) weeks between first (1st) and third (3rd) dose (112 days). Not required for Kindergarten through 5th grade: if a student has begun the 3-dose series, it is recommended that they complete the series according to the scheduling intervals.
MMR - Two (2) doses, first (1st) administered on/after the first (1st) birthday, second (2nd) dose no less than four (4) weeks after the first (1st) dose.
VARICELLA (Chickenpox) - Two (2) doses entering Kindergarten: administered on/after the first (1st) birthday.
Meningococcal (MCV4) – 6th grade (and 7th & 8th grade transfer students): One (1) dose on or after the 11th birthday.
Tdap - 6th grade (and 7th & 8th grade transfer students): One (1) dose on or after the 10th birthday.
The dates of all immunizations must be noted on the Certificate of Child Health Examination Form (including month, day, and year).

MUMPS - One (1) dose, administered on/after the first (1st) birthday.

Hib - Follow the appropriate schedule. Children twenty-four (24) to fifty-nine (59) months of age who have not received a primary series according to the Hib schedule, must show proof of receiving one (1) dose at fifteen (15) months of age or older.

Kindergarten is an exciting time, and we look forward to being a part of your child’s learning journey!. We want to ensure the transition goes as smoothly as possible, and one of those steps is completing all health requirements for Kindergarten.
**The following are required BEFORE entrance into Kindergarten:
All Illinois children are required to have a physical health examination within one year before entering school:
The health examination is valid if performed within one year of the first day of school, beginning from August 19, 2025.
The Health History section found on the physical exam form MUST be completed, signed, and dated by a parent
Upon entry into Kindergarten, students must show proof of up-to-date vaccinations, which must meet state mandates, including # of doses, age, and interval requirements:
The examination must be performed within one year of the first day of the school year.
Action Plans:
For students with a medical condition (such as food and latex allergies, asthma, diabetes, seizures, POTS, EOE), Action Plans are required to be on file in the nurse’s office by the first day of school. Action Plans should be provided and completed by your child’s physician. For students with food allergies, please also complete the food allergy questionnaire.
Daily Medication:
For any student requiring daily medication to be given at school, the following forms must be completed by their doctor and parent for each medication. Authorization forms should be dated after June 30th, 2026.
As-Needed Medication:
For any student requiring as-needed medication (such as EpiPens/Auvi-Q, Benadryl, asthma inhalers, insulin, Tylenol, Ibuprofen), the following forms must be completed by the doctor and parent for each medication. Authorization forms should be dated after June 30th, 2026.
The Illinois State School Code Health Law, Section 665, requires that a child have a complete physical examination, including mandated immunizations, upon entrance into kindergarten and sixth grades.
Any child entering sixth grade shall show proof of receiving one dose of Tdap vaccine on or after the 11th birthday, regardless of the interval since the last DTap, DT, or Td dose.
Any child entering the sixth grade shall show proof of having received one dose of Meningococcal Conjugate vaccine on or after the 11th birthday.
Any child entering 6th grade is required to show proof of having 3 doses of Hepatitis B. The first two doses shall have been received no less than 4 weeks (28 days) apart. The interval between the second and third doses shall be at least 2 months (56 days). The interval between the first and third doses shall be at least four months (112 days).
The immunization record must also show proof of 2 MMR doses and 2 Varicella doses.
The physical examination must be administered within 12 months before the child’s first day of entry into school.
If your child plans to participate in interscholastic sports, they will require a current “sports physical.” They are only valid for 395 days from the date of the physical. Students participating in athletics may need to complete a new physical during the school year, depending on the date of their last exam.
The only difference between a sports physical and a regular physical is that a sports physical does not require immunization records.
Since your child will enter sixth grade in the fall, a “Certificate of Health Examination” is required and is to be completed by you and your physician. The physical exam has 2 pages and requires 3 signatures. The signatures required are:
1. Immunizations (health care provider)
2. Health history (parent)
3. Physical exam signed by MD, APN, or PA
A dental examination is required for all students entering kindergarten, second grade, and sixth grade.
It is recommended that parents make arrangements with their physicians as soon as possible to ensure the required physical examination(s) and immunizations before registration in August.
Failure to comply with the above statutory requirements will result in your child’s exclusion from school.

Please keep your child(ren) home when they are sick or not feeling well enough to be in school and consult your medical provider.
Inform your school nurse of your child(ren)'s symptoms and any diagnoses your child(ren) has received (i.e. Strep Throat, Conjunctivitis, Influenza, Covid, etc.)
Your school nurse will provide information on when your child(ren) may return to school, specific to his/her (their) symptoms and/or diagnoses.
If your child is absent due to illness, a physician's note is required after three consecutive absences.
To be excused from P.E. for more than 3 days, a physician's note must be on file in the school health office. The note must include the reason for the restriction and the length of time the student is unable to participate in some or all activities. If the duration is not specified, a second physician's note indicating when the student may return to P.E. will be required.

The school district includes informational materials regarding influenza and influenza vaccinations and meningococcal disease and meningococcal vaccinations developed, provided, or approved by the Department of Public Health under Section 2310-700 of the Department of Public Health Powers and Duties Law of the Civil Administrative Code of Illinois when the board provides information on immunizations, infectious diseases, medications, or other school health issues to the parents or guardians of students.
As described in 105 ILCS 5/22-30, the school district is participating in the Undesignated Medications Program in collaboration with the Lake County Health Department (LCHD). In this Program, the Medical Epidemiologist at LCHD has issued a standing order protocol for trained school personnel to use undesignated medications [i.e. epinephrine injection, asthma medication, and/or naloxone] in the setting of medical emergencies wherein a student, visitor, or staff person is experiencing severe allergic/anaphylactic reaction, respiratory distress, and/or opioid overdose. The medications will be administered in appropriate circumstances.
As described in 105 ILCS 5/22-30, the school district, public school, charter school, or nonpublic school and its employees and agents, including the Lake County Health Department Medical Epidemiologist, are to incur no liability or professional discipline, except for willful and wanton conduct, as a result of any injury arising from the administration of epinephrine, asthma medication, and opioid antagonists regardless of whether authorization was given by a pupil's parents or guardians or by a pupil's physician, physician assistant, or advanced practice registered nurse. Parents or guardians must indemnify and hold harmless the school district, public school, charter school, or nonpublic school and its employees and agents, including the Lake County Health Department Medical Epidemiologist, against any claims, except a claim based on willful and wanton conduct, arising out of the administration of asthma medication, an epinephrine injector or an opioid antagonist regardless of whether authorization was given by the pupil's parents or guardians or by the pupil's physician, physician assistant, or advanced practice registered nurse.
